The case for and against

Updated Oct 14, 2026

Bull case

Cigna Group is well positioned because its earnings base is increasingly anchored by higher-margin, more defensible businesses in Evernorth specialty and in Cigna Healthcare’s Select segment, where self-funded smaller employers remain an underpenetrated growth pool. Management’s 10.0%-14.0% long-term adjusted EPS CAGR and at least $45.00 adjusted EPS floor by CY30 appear credible given the $3.0B enterprise efficiency program, roughly $50.0B of 5-year cash flow guidance, and identifiable growth drivers such as Select expansion, International, and Stop Loss repricing. Although Pharmacy Benefit Services faces contracting transition risk, the company’s scale in US and international medical membership, plus its winning of the Centene deal, supports a favorable outlook on durable earnings growth.

Bear case

Cigna Group is facing a challenging earnings transition as Evernorth’s Pharmacy Benefit Services shifts to the rebate-free Signature model, with margins already guided down to 3.0-3.5% from 3.5-4.5% and PBS growth only flat to 4.0% through CY30. The company’s ambitious 10.0-14.0% adjusted EPS CAGR target and at least 10.0% EPS growth in CY27 look difficult to underwrite given high execution risk, potential customer resistance to higher administrative fees, and uncertainty around network contracting and medical cost trends. Although specialty businesses like CuraScript are growing quickly and the company has scale with 16 million US and 2 million international medical members, the dependence on a large PBS reset and concentrated contract risk creates downside if growth or margins disappoint.
