The case for and against

Updated Oct 14, 2026

Bull case

DexCom is positioned for durable fundamental growth because its CGM platform is extending beyond intensive diabetes care into the large Type 2 non-insulin and broader consumer markets, where CONNECT showed a −1.6% A1c reduction versus −0.7% control, ~21% TIR improvement, and ~97% adherence. Its software-led Stelo experience, with frictionless meal logging, daily insights, and AI coaching, suggests retention and utilization can be driven by engagement rather than sensors alone, supporting a differentiated ecosystem and a scalable, profitable model. Financially, the company is already showing operating leverage, with adjusted gross margin of 63.5%, adjusted operating margin of 22.2%, adjusted EBITDA margin of 30.6%, and $2.42B of cash and marketable securities, while 2026 revenue guidance of $5.16B-$5.25B implies 11-13% organic growth.

Bear case

DexCom is facing a weakening fundamental case because sensor performance still has meaningful limitations, with compression lows during sleep or when lying on the sensor arm reducing overnight glucose-analysis reliability and creating a usability gap versus the company’s claims of convenience. Its outlook is also pressured by sector-level risks such as regulatory delays, pricing pressure from payors and CMS, and the high capital investment required to compete, all of which could make profitability difficult even if CGM adoption expands. In addition, the stock’s significant premium multiple leaves little room for disappointment if CGM penetration stalls, competitive products erode its moat, or new product launches are delayed.
