The case for and against

Updated Oct 14, 2026

Bull case

Encompass Health is attractive because it operates the largest inpatient rehabilitation platform in a structurally undersupplied market, where the number of IRFs has slipped from 1,179 in 2010 to 1,170 in 2024 while the 65+ population has risen to over 18%. Its 2Q results showed adj EBITDA up 9% and same-site volume up 2.8%, helped by 3.9% net revenue per discharge growth from higher-acuity stroke and brain injury cases, and management lifted the EBITDA guide by $15m at both ends. The outlook is further supported by low leverage of 1.8x, strong free cash flow, and a scalable expansion pipeline, including 18 planned or underway projects and potential North Carolina de novo growth beginning in 2028-29.

Bear case

Encompass Health is vulnerable to a negative fundamental setup because its growth thesis depends on rapid volume expansion, de novo ramp, and bed additions that may underdeliver while staffing costs and turnover keep pressuring margins. Regulatory and payer risks are material: CMS could cut IRF reimbursement, MedPAC has suggested a -7% rate cut with an estimated -18% adjusted EBITDA impact, and Medicare Advantage plans are increasing denials and admission pressure. Although revenue per discharge rose 3.9% in 2025 and 1Q26 pricing remained solid, those gains may decelerate as reimbursement normalizes and higher maintenance capex, potentially peaking near 15% of revenue over the next 2-3 years, diverts cash from expansion and shareholder returns.
