The case for and against

Updated Oct 14, 2026

Bull case

Guardant Health is benefiting from a rare combination of category leadership, reimbursement expansion, and product adoption momentum across liquid biopsy, MRD, and screening. Its flagship Guardant360 franchise has generated strong volume growth, while Reveal is gaining traction from CMS reimbursement and Shield has added major upside from NCCN and ACS guideline inclusion, UNH commercial coverage effective August 1, 2026, and a $1,495 price point. Fundamentally, the business also has substantial operating leverage, with Q1’26 revenue of $302M (+48% Y/Y), gross margin of 65%, $1.2B cash, and multiple catalysts that could lift ASPs and volumes through 2026 and beyond.

Bear case

Guardant Health is facing a fundamental valuation and execution problem because its core Guardant360 franchise still represents more than 60% of sales, making the business highly exposed if growth decelerates from recent 30% Y/Y volumes toward the sub-20% range. Although Shield posted a strong Q1'26 with about $42M of revenue and roughly 44k volumes, the franchise still relies on Medicare-rich pricing of $1,495/test through YE'27, and management expects per-test revenue to fall as commercialization broadens, while cash burn has exceeded $200M in each of the past two years. Meanwhile, downside risk is amplified by uncertainty around the true size of the blood-based screening market, dependence on future USPSTF support, and execution risk around a path to breakeven by Q4 2027.
