The case for and against

Updated Oct 14, 2026

Bull case

Gilead Sciences is attractive because its HIV franchise remains highly durable, with Biktarvy, Descovy, and Yeztugo showing strong uptake and even a raised FY26 revenue guide to $1B for Yeztugo and an 8% HIV growth outlook. Its commercial moat is being widened by longer-acting and oral PrEP options, including the FDA acceptance of 300 mg QW oral Yeztugo for a Feb 2, 2027 PDUFA, which could strengthen share gains and support a unified brand strategy. Beyond virology, the company adds upside through oncology assets like Yescarta, Tecartus, Trodelvy, and anito-cel, creating long-duration growth optionality while sustaining meaningful cash flows from its core businesses.

Bear case

Gilead Sciences is facing a fundamentally pressured outlook because its core HIV and HCV franchises remain exposed to generic entrants, pricing pressure, and potential U.S. government cuts to HIV treatment funding, while competition could further erode growth and margins. Although Yeztugo benefits from 95% coverage and very strong +14% y-o-y PrEP market growth, the $158M U.S. print, weaker-than-hoped persistence, and a more conservative $3.9B peak sales view versus $7B consensus suggest limited confidence in long-term PrEP expansion. The pipeline does not fully offset these risks, as bic/len lacks clear advantages versus Biktarvy, anito-cel may have a limited initial role, and Trodelvy’s EVOKE-3 and broader oncology execution remain uncertain.
