The case for and against

Updated Oct 16, 2026

Bull case

GRAIL is supported by a differentiated clinical profile for Galleri, with studies showing 99.6% specificity, over 90% cancer signal origin accuracy, 60% positive-predictive value in PATHFINDER 2, and a 14% reduction in Stage IV cancers in NHS-Galleri. Its extensive dataset, including over 300K tests in studies and trials, strengthens the case for FDA approval and helps position the test as a potential gold standard in a large, underpenetrated MCED market. Near-term catalysts such as FDA approval, Medicare coverage, and broader guideline adoption could unlock stronger revenue growth, while 1Q revenue of $40.8M, 28% total sales growth, and $823M in cash underscore improving commercial momentum.

Bear case

GRAIL is viewed negatively because it has incurred significant net losses since inception and is expected to keep burning cash while funding lengthy, expensive clinical studies, regulatory filings, and commercialization efforts that may require additional financing. Its Galleri test faces major adoption hurdles: FDA has never approved an MCED test, broader government reimbursement is not yet in place, and the self-pay market at a $949 cash-pay price appears structurally limited with weak conversion even among affluent, health-motivated patients. On top of that, the company depends on sole suppliers, limited third-party service providers, and may face rising costs as the Illumina royalty resumes in 2027, leaving upside constrained by slow volume growth and continued OpEx above revenue.
