The case for and against

Updated Oct 14, 2026

Bull case

iRhythm Holdings is viewed positively because its Zio platform combines a wire-free, 14-day wearable biosensor with proprietary cloud analytics, creating a differentiated, scalable solution that can improve physician workflow and patient diagnosis while supporting continued volume growth. Recent performance reinforces that thesis, with revenue of $224.2M up 20.1% year over year, gross margin of 72.8%, and adjusted EBITDA of $43.3M at a 19.3% margin, showing both operating leverage and disciplined execution. Longer term, the company’s 2027 outlook calls for revenue above $1B and greater than 70% gross margin, while the VitalConnect acquisition should expand its addressable market, add at least $65M in next 12-month revenue, and strengthen its competitive position in ambulatory cardiac monitoring.

Bear case

iRhythm Holdings is facing a deteriorating fundamental setup because the DOJ Civil Investigative Demand and the June 2023 FDA warning letter together create regulatory uncertainty that could weigh on Zio AT reimbursement, commercialization, and investor confidence. Reimbursement risk is also substantial, as the company faces scrutiny over rate reductions, prior authorization, coverage limitations, and managed care pressure, while rising competition from larger device firms and emerging entrants threatens market share and execution. Even if the CID is ultimately resolved with a manageable settlement, the unresolved FDA issues, potential adverse settlement terms, and the risk of underwhelming product launches or an additional equity offering keep the outlook negative.
