The case for and against

Updated Oct 21, 2026

Bull case

MoonLake is attractive fundamentally because SLK has shown durable, broad-based efficacy across hidradenitis suppurativa and psoriatic arthritis, including 67.2% HiSCR75 and 33.1% HiSCR100 at Week 52, while also delivering meaningful patient-reported improvements and no new safety signals in IZAR-1. Its IL-17A/F-targeted Nanobody design and apparent comparability to Bimzelx support a differentiated mechanism that could be highly competitive in inflammatory skin and joint disease, especially as additional IZAR data and axSpA development progress in the coming months and before the end of this year. Financially, the company bolstered its balance sheet with a $230M offering, which may support self-commercialization and reduce near-term dilution risk while the BLA for HS was submitted on September 30 and the current enterprise value of less than $700M appears disconnected from the clinical profile.

Bear case

MoonLake is viewed pessimistically because its valuation depends on a highly uncertain clinical and regulatory path for SLK, with approval assumptions already only 60% to 70% in HS and 40% to 60% in PsA and axSpA. The DCF support is sensitive to a 12% to 15% discount rate, 3% terminal growth, and roughly 87M to 88M fully diluted shares as of mid-2027, underscoring how much future value relies on execution rather than current fundamentals. Key risks cited—timely pivotal trial progress, compelling efficacy, regulatory submission delays, major market approvals, competition, slower uptake, partnership risk, and long-term dilution—collectively justify the negative outlook.
