The case for and against

Updated Oct 14, 2026

Bull case

Monopar Therapeutics is viewed favorably because ALXN1840 has built a compelling late-stage Wilson disease case, with a Phase 3 trial meeting its primary endpoint, 645 patient-years of follow-up in 266 patients, and long-term data showing sustained copper control, neurological improvement, hepatic stabilization, and manageable safety. The July initiation of a rolling NDA submission, together with Fast Track, Orphan Drug, and Rare Pediatric Disease designations, improves regulatory optionality and may support a faster review path, while cash of $134.3M is said to fund operations through at least YE27. Beyond ALXN1840, the MNPR-101 radiopharmaceutical platform for imaging and treating uPAR-expressing cancers adds pipeline upside and makes the story more than a single-asset thesis.

Bear case

Monopar Therapeutics is a clinical-stage biopharmaceutical company whose value depends on unproven assets, making the investment thesis highly fragile if ALXN1840 or its radiopharmaceutical pipeline fails to show meaningful efficacy, safety, or statistical significance in current or future clinical trials. Regulatory risk is also elevated because FDA and other agencies can demand additional mechanistic studies, change leadership views, or delay or deny approval, while reimbursement uncertainty could prevent any approved therapy from generating revenues. The company also faces meaningful financing and competitive pressures, since it may need additional equity financing and competes with multiple Wilson disease programs and better-resourced rivals, limiting commercial adoption even if development succeeds.
