The case for and against

Updated Oct 16, 2026

Bull case

Neurogene is supported by a strengthened balance sheet, with $225.4M in cash, cash equivalents and short-term investments at June 30 and about $360.2M pro forma cash after the ~$134.8M net proceeds from the follow-on offering, extending runway into 1Q29. That funding should carry the company beyond Embolden top-line data in 2H27 and through PPQ completion by YE26, reducing financing risk while advancing NGN-401 toward a potential BLA. Fundamentally, the positive view is anchored in NGN-401’s compelling Phase 1/2 efficacy and durability—8/10 patients met the pivotal composite endpoint at 12 months, 10/10 improved on CGI-I and gained milestones, and safety at the 1E15 vg dose has remained supportive across 35 treated patients.

Bear case

Neurogene is facing a highly adverse risk profile because its lead program is exposed to both broad biotech execution hazards and company-specific failures, including development, regulatory, commercial, manufacturing, financing, and IP risk. Safety is especially concerning after cases of HLH/hyperinflammatory syndrome at the current clinical dose of 1E15 vg, while efficacy remains uncertain because the registrational trial could require a 35% response rate versus 80% observed to date. Regulatory and adoption risks further weaken the investment case, as FDA alignment on using the ongoing single-arm EMBOLDEN study for BLA support was reversed and ICV delivery may be a larger barrier than assumed, especially in a non-degenerative disease like Rett syndrome.
