The case for and against

Updated Oct 21, 2026

Bull case

NeuroSense Therapeutics is supported by PrimeC’s differentiated multi-target mechanism, which addresses shared neurodegenerative drivers such as neuroinflammation, autophagy dysfunction, miRNA imbalance, and iron dyshomeostasis rather than a single pathway. Phase 2b PARADIGM in ALS provided the strongest evidence, showing favorable tolerability, statistically significant TDP-43 reductions versus placebo, a 7.92-point ALSFRS-R advantage at month 18, and a 36.3-month median survival versus 21.4 months, while AD biomarker data from RoAD in 2Q26 further broaden the platform’s appeal. The outlook is tempered by financing risk, as the company ended 2025 with $0.17 million in cash and faces Nasdaq compliance issues, but the planned 4Q26 Canada filing and 1H27 PARAGON initiation create meaningful clinical and regulatory catalysts.

Bear case

NeuroSense Therapeutics is a highly speculative clinical-stage biotech whose value hinges almost entirely on PrimeC, while its limited cash position and the threat of dilutive financing or failed funding create immediate balance-sheet risk. Although PrimeC has shown encouraging ALS signals, including an 18% relative reduction in ALSFRS-R decline, a 30% relative reduction in FVC deterioration, and biomarker improvements in TDP-43, LC3, and miRNAs, these results remain from small, non-definitive studies and have not yet been validated in registrational trials. The company also faces listing and execution risk, with first-patient enrollment for the pivotal ALS trial still contingent on securing strategic resources and commercial adoption uncertain even if approval is achieved.
