The case for and against

Updated Oct 14, 2026

Bull case

Oculis Holding is attractive fundamentally because licaminlimab (OCS-02) has already produced encouraging Phase 2 DED results, including statistically significant symptom relief in 134 patients and a biomarker-enriched objective sign signal in RELIEF, with favorable tolerability and no treatment-related serious ocular safety signal. The investment case is further strengthened by PREDICT-1, an actively enrolling ~160-patient precision-medicine study targeting the TNFR1 genotype, which could validate a differentiated, genotype-guided DED approach and support meaningful commercial potential with peak annual sales modeled at $1.7B and 55% probability of success. In parallel, privosegtor (OCS-05) has FDA Breakthrough Therapy designation and SPA alignment for optic neuritis, a rare sight-threatening condition with no approved neuroprotective treatments and a potential $7B U.S. market, while the company’s ~$278MM cash position funds operations into 2H29.

Bear case

Oculis Holding is viewed negatively because its lead DME program, OCS-01, failed both ph 3 DIAMOND-1 and DIAMOND-2 primary endpoints in wk52, and the company has already said it will not pursue a regulatory filing for that indication. The remaining pipeline still carries meaningful binary risk, with only 45% approval probabilities assigned to OCS-02 in DED and OCS-05 in AON, while the valuation itself depends on discount-rate-heavy DCF assumptions, $277.6M to $282.3M of cash, and 60.4M to 61.8M shares outstanding by 2Q27. Beyond clinical setbacks, operating expenses are set to rise for late-stage development, additional capital will likely be needed, and reliance on third-party manufacturing adds supply and tariff exposure.
