The case for and against

Updated Oct 15, 2026

Bull case

OKYO Pharma is attractive because urcosimod is advancing in the Phase 3 NEPTUNE trial for neuropathic corneal pain, a chronic condition with no FDA-approved treatment options and therefore a substantial unmet medical need. The FDA Type D meeting was positive, and the trial design, with about 111 adults and a Week 12 pain endpoint where a ≥2-point placebo-adjusted improvement may be clinically meaningful, supports a potentially de-risked registrational path; management also expects enrollment completion in 1H27 and topline data in 2H27. Supportive March 2026 Phase 2a results, including a 2.75-point pain delta and broader quality-of-life improvements, strengthen confidence in urcosimod, while the company’s existing capital may fund the pivotal program through topline data.

Bear case

OKYO Pharma is viewed negatively because its valuation rests on a single clinical-stage asset, urcosimod, with only modest approval odds cited across indications and a DCF-based asset value that already excludes the cash position. The company faces concentrated binary risk from potential clinical failure, U.S. regulatory rejection, and weak commercial uptake in a competitive market, while the recurring mention of dilution risk signals pressure on shareholders if additional capital is needed. Even under the stated assumptions of 43M shares at the end of March 2026 or 62.5M shares at the end of 2Q27, the investment case remains highly speculative and fragile.
