The case for and against

Updated Oct 21, 2026

Bull case

Personalis is attractive because its NeXT Personal MRD test has achieved strong clinical traction, with Q1'26 molecular clinical test volume of 7,815, up 258% Y/Y and 26% q/q, while over 1,000 oncologists were ordering the assay and Medicare coverage wins improved reimbursement visibility. The company also has a growing biopharma MRD business tracking to about $20M in 2026 and valuable sequencing and tumor-profiling capabilities that support personalized cancer vaccines, adding diversified revenue streams beyond clinical diagnostics. Although gross margin compressed from 32% in 2024 to 23% in 2025 and 2% in Q1'26 as commercialization costs were absorbed, the strategic combination with Tempus should provide greater scale, a larger sales force, and a path to accelerate growth in a very large MRD market.

Bear case

Personalis is viewed negatively because its economics remain weak, with Q1’26 gross margin at just 2% and operating losses continuing despite $15.5M of revenue and $233M of cash, underscoring heavy burn and limited near-term profitability. The company’s growth story is also fragile because biopharma ordering can be volatile quarter-to-quarter, large pharma demand for personalized cancer vaccines may soften, and reimbursement for its MRD test may come in below expectations. Even with the announced TEM acquisition, the stock trades very close to deal value, leaving little upside while execution risks, partner dependence, and the possibility that the deal does not close keep the risk/reward unattractive.
