The case for and against

Updated Oct 17, 2026

Bull case

PTC Therapeutics is supported by a strong commercial base from Sephience, which launched in 2025 and has already outperformed internal projections and investor expectations, with $124.6M in revenue and over 90% of U.S. centers of excellence having prescribed it as of 1Q26. The company also has meaningful incremental upside from ST-920 in Fabry disease, acquired for $111M upfront plus up to $100M contingent on regulatory milestones, backed by favorable 52-week renal data and a large ~11,000-patient U.S. addressable population. Beyond that, votoplam adds further optionality through positive Phase 2 data, while PTC’s broader rare-disease pipeline, global expansion plans to more than 30 countries by end-of-2026, and evidence of durable switching and low double-digit discontinuations support a longer growth runway.

Bear case

PTC Therapeutics is weighed down by heavy dependence on a narrow set of rare-disease launches, making its revenue base vulnerable if Sephience underperforms, Translarna weakens after the European Marketing Authorization withdrawal, or Emflaza and DMD products miss guidance. The company also faces elevated clinical, regulatory, and reimbursement risk because rare-disease enrollment delays can materially push out sales, AAV gene therapy still has limited U.S. approval history, and payors may resist some of the most expensive treatments. Even where the pipeline has promise, the market is already assuming meaningful value from Sephience and votoplam, so any slower uptake, development setback, or commercial shortfall could force a sharp reassessment of the stock’s fundamentals.
