The case for and against

Updated Oct 21, 2026

Bull case

SAB Biotherapeutics is attractive because Tzield’s accelerated approval in Stage 3 T1D validates preservation of C-peptide as an approvable endpoint and de-risks SAB-142’s regulatory path. The company’s lead asset has a differentiated profile versus rATG and teplizumab, with early Phase 1 data showing no anti-drug antibodies after redosing through Day 360, preserved Tregs, and predictable PK that supports a 1.5 mg/kg every 6 months maintenance regimen. With an estimated ~1.5 million U.S. T1D patients, ~64,000 new cases per year, and modeled 2035 revenues of ~$2.4 billion, the commercial opportunity appears substantial if the Phase 2b SAFEGUARD study confirms these signals.

Bear case

SAB Biotherapeutics is burdened by a highly speculative, single-asset profile, with valuation tied almost entirely to SAB-142 while the FDA already stopped short of a broader label after key secondary endpoints of PROTECT failed to show reduced HbA1c or lower insulin needs. The company remains clinical-stage, has a history of losses, and expects significant operating losses to continue because research, development, clinical testing, and general and administrative costs will stay high, increasing the need for dilutive equity or restrictive debt financing. Its outlook is further weakened by limited clinical data, regulatory uncertainty, competition from better-resourced peers, no sales or distribution infrastructure, and unique operational risk from maintaining approximately 200 genetically engineered animals at a single South Dakota location.
