The case for and against

Updated Oct 15, 2026

Bull case

Tactile Systems Tech is well positioned because it serves large, under-penetrated chronic-care markets with differentiated home-based therapies, including Flexitouch, Nimbl, and AffloVest, while benefiting from structural tailwinds such as improving diagnosis, society guidelines, and the Medicare NCD. The company’s recent operating transformation in FY25, combined with a revamped sales force, CRM-enabled workflow improvements, and rising territory productivity, is translating into strong execution, as shown by 1Q26 revenue of $75.3M, lymphedema growth of 23%, and Airway Clearance growth of 22%. Fundamentals also look attractive with gross margins of 76.5%, FY26 revenue guided to $360M-$368M, and valuation near 1.4x NTM EV/Sales versus peers at 3.5x, supporting confidence in durable growth.

Bear case

Tactile Systems Tech is facing a fundamentally challenged outlook because growth depends on faster uptake in commercial channels while Medicare documentation burdens, coverage confusion, and varying physician opinions on pneumatic compression can slow adoption. The company also faces execution risk from sales force productivity limits, as 2Q26 ended with 332 reps, down from 335 in 1Q26, leaving reps burdened with non-selling tasks instead of revenue generation. Although management maintained adjusted EBITDA guidance of $49M to $51M and expects ~9% revenue CAGR through FY28, higher G&A, ~11% operating expense growth, and Airway Clearance headwinds pressure margins and make the valuation harder to justify.
