The case for and against

Updated Oct 16, 2026

Bull case

Tempus AI is well positioned because its FDA-cleared ECG-PH product expands the cardiovascular portfolio to a third cleared offering, broadening the addressable market and reinforcing the scalability of its ECG-AI platform. Its positive outlook is further supported by the pending Personalis acquisition, which gains strategic validation from Merck and Moderna’s Phase 3 INTerpath-001 readout, while Personalis’ MRD coverage expansion and ASPs rising from about $139 in 4Q25 to $245 in 2Q26 strengthen the commercial case. Tempus also has a near-term earnings catalyst in xT CDx, with management expecting about $200 of ASP improvement per test and roughly $85 million of annual revenue beginning in 2027, alongside the possibility of about $400 million of incremental revenue in 2028 from unified ADLT pricing across xT and xF.

Bear case

Tempus AI is facing meaningful integration and execution risk from the Personalis acquisition, with NeXT Personal still in an early commercial ramp and only four MolDX coverage decisions in place, while no commercial payer contracts have been disclosed. Medicare coverage alone does not assure broader adoption because commercial payers represent about 50% of the eligible patient population, and their coverage decisions often lag MolDX by 12–24 months, extending the reimbursement overhang. Clinical and competitive uncertainty further दब दब दब?
