The case for and against

Updated Oct 14, 2026

Bull case

Veracyte is attractive because its core franchises—Afirma and Decipher—have durable clinical leadership, strong reimbursement support, and meaningful penetration headroom, with Decipher backed by Simon Level 1B evidence and 2026 NCCN inclusion, while Afirma continues to expand volume and coverage into Bethesda V. The company also shows unusual operating quality for diagnostics, having generated $517.2M of revenue in FY2025, 27.56% adjusted EBITDA margins, $136.3M of operating cash flow, and $439.1M of cash with no debt as of March 31, 2026, supporting self-funded growth and an expanding margin profile. Its pipeline adds further upside through TrueMRD, Prosigna, and Decipher Bladder, each leveraging the same platform and commercial infrastructure to broaden addressable markets and create a multi-year runway for high-margin growth.

Bear case

Veracyte is fundamentally exposed to concentration risk because revenue still depends heavily on Decipher and Afirma, while community urologist adoption of molecular profiling remains slow and the thyroid testing market is already mature with only low-single-digit growth. Reimbursement and payer pressure are a major drag, as Medicare reached about 20% of FY2025 revenue and UnitedHealthcare about 14%, increasing vulnerability to CMS, MolDX, LBMs, prior authorization, and rate cuts that can hurt both volume and pricing. At the same time, concurrent launches and pipeline expansion are pressuring profitability, with Q1 2026 R&D rising 53% to $27.1M, while competitive threats, regulatory uncertainty, and unsettled serial-MRD economics raise the risk of underperformance in sales, margins, and free cash flow.
