The case for and against

Updated Oct 7, 2026

Bull case

Vivos Therapeutics is attractive because its non-surgical, non-pharmaceutical OSA platform addresses a large unmet need with better compliance potential than CPAP, while the company’s position in a market affecting over 30 million U.S. adults supports meaningful long-term expansion. Operationally, revenue grew 16.0% in 2025 to $17.4 million, and gross margin improved sharply to 78% in Q4 from 58% in Q3, suggesting improving economics as the channel expands through sleep centers and partnerships. Despite a 2025 net loss of $21.2 million and execution risks from provider attrition and capital needs, the combination of higher margins, cost controls, and accretive acquisition opportunities supports a constructive outlook.

Bear case

Vivos Therapeutics is facing a difficult transition as Q4 FY25 revenue fell to $3.8 million versus a $7.3 million estimate, with the decline tied to weaker product sales while shifting from the legacy VIP/dentist model to the sleep-center model. The new strategy is proving operationally slow and costly, since management cited prescriber hiring constraints, operating expenses rose 90% year over year to $9.4 million, and net loss widened to $7 million. Liquidity remains strained, with $2.0 million of cash at the end of December 2025, $8.8 million of debt, $15.3 million of operating cash burn during 2025, and share count increasing to 13.5 million after roughly $6.8 million of subsequent financings, signaling continued dilution and financing risk.
