The case for and against

Updated Oct 15, 2026

Bull case

GeneDx Holdings is supported by a favorable reimbursement and adoption backdrop, as Carelon’s outpatient genome coverage policy moves whole genome sequencing toward a first-line, medically necessary option when criteria are met, reducing reimbursement uncertainty and encouraging an exome-to-genome mix shift. That shift already lifted WGS volumes to 47% of Q4’25 volumes from 43% in Q3’25 and should support higher ASPs and gross margins, with the policy potentially adding about $100 in incremental ASP by H2 2027. The company also has multiple growth levers from 2026 execution, including 30%+ volume growth in WES/WGS, a near tripling of the sales force to ~160 reps, new product launches, and the Fabric Genomics acquisition, which can broaden commercialization and improve long-term profitability.

Bear case

GeneDx Holdings is facing a weakened fundamental backdrop as Q1'26 revenue of $102M missed expectations, adjusted gross margin of 68.5% trailed consensus, and adjusted net income was negative $8M, despite WES/WGS volumes rising 34% Y/Y to 27,488. Its outlook also deteriorated when it cut 2026 revenue guidance by $65M to $475M-$490M and reduced core exome/genome growth expectations, suggesting the business is still highly exposed to reimbursement, mix pressure, and uneven non-core demand. The fact that the miss came from five separate buckets, alongside concerns about guidance reliability and a projected 2028 adjusted net income of only $25M versus a prior $82.5M estimate, supports a cautious view.
