The case for and against

Updated Oct 21, 2026

Bull case

Surrozen is supported by a compelling scientific rationale, as its lead asset SZN-8141 combines Wnt activation with VEGF inhibition to target both vascular repair and leakage in retinal disease, potentially improving efficacy and durability versus single-agent anti-VEGF therapy. Positive read-through from Merck’s BRUNELLO trial in DME strengthens the case for Wnt biology, while Surrozen’s preclinical data showed accelerated vascular regeneration and reduced avascular and neovascular tuft areas in OIR and sCNV models, with observed superiority versus single agents and anti-VEGF. The company also has near-term catalysts, including IND submission anticipated by the end of 3Q26, Phase 1b/2a initiation by year-end 2026, and a PTAB decision reinforcing its patent position, all while management raised modeled DME development probability to 20% and targets greater wet AMD penetration.

Bear case

Surrozen is a clinical-stage biotech with promising Wnt-pathway biology, but the investment case is weakened by unresolved safety concerns around remigromig, including a higher incidence of PDR, vitreous hemorrhage, and AE-related discontinuations that could limit clinical and regulatory confidence. Its financial profile also remains fragile, with 1Q26 R&D of $9.3M, G&A of $6.1M, a net loss of $127.5M, and cash and equivalents of just $106.9M as of March 31, 2026, underscoring ongoing capital dependence. Even with platform progress toward SZN-8141 IND submission anticipated 2H26, the thesis remains highly exposed to development setbacks, commercialization uncertainty, and the risk that peak revenue assumptions prove too optimistic.
